Je ne pense pas que cette étude va révolutionner les choses pour nous mais en tt cas j'y vois qd même un constat des incertitudes qui entourent le diagnostic de neuroborréliose.

BACKGROUND: No consensual criteria exist to diagnose neuroborreliosis. The intrathecal anti-Borrelia antibody index (AI) is a necessary criterion to diagnose neuroborreliosis in Europe, but not in the United States. Previous studies to determine the diagnostic value of the AI found a sensitivity ranging from 55% to 80%. However, these studies included only typical clinical cases of meningitis or meningoradiculitis, and none had a control group with CSF anti-Borrelia antibodies. METHODS: We studied a sample of 123 consecutive patients with clinical signs of neurologic involvement and CSF anti-Borrelia antibodies. We determined the AI for all patients and a final diagnosis was made. Patients were then divided into three groups (neuroborreliosis, possible neuroborreliosis, control). RESULTS: Thirty of the 40 patients with neuroborreliosis had a positive AI (AI sensitivity = 75%). Two of the 74 patients with another neurologic diagnosis had a positive AI (AI specificity = 97%). CONCLUSION: The antibody index has a very good specificity but only moderate sensitivity. Given the lack of consensual criteria for neuroborreliosis and the absence of a "gold standard" diagnostic test, we propose pragmatic diagnostic criteria for neuroborreliosis, namely the presence of four of the following five items: no past history of neuroborreliosis (!!!!!!!!), positive CSF ELISA serology, positive anti-Borrelia antibody index, favorable outcome after specific antibiotic treatment, and no differential diagnosis. These new criteria will need to be tested in a larger, prospective cohort.

En 2 mots... cela veut t-il dire qu'à Stras, ils n'auraient jamais vu de neuroborréliose????

Non, c'est plus drôle : l'article commence par dire qu'il n'existe pas de critères de diagnostic bien définis pour la borreliose. (Pourquoi ce fi̺î¬fiÏfi¬Ïfi¬ de concensus de 2006 alors ?).

La phrase qui m'a fait rebondir, c'est celle qui dit que pour diagnostiquer une borreliose à quelqu'un, il faut que celui-ci n'en ai jamais eu une auparavant…

Là je m'interroge du sérieux de cette phrase quand même, ou alors, ils utilisent Google translate à Strasbourg parce qu'ils sont trop popo en anglais… :yeah

Aucun critere n’est consensuel à l’heure actuelle afin de diagnostiquer une neuroborreliose. L’index intrathecal (IA) anti- borrélia est un critère necessaire pour en faire le diagnostic en Europe, mais non aux US . les etudes anterieurement menées pour calculer la valeur de diagnostic de l’IA ont trouvé une sensibilité allant de 55% à 80%. Cependant ces études incluaient seulement des cas cliniques typiques de menengites ou de meningoradiculites , et aucunes ne comportaient de groupes temoins avec des anti –corps anti borrelia dans le liquide cephalorachidien ( LCR).
Methode entreprise :
Nous avons étudiés un echantillon de 123 patients avec des signes cliniques neurologiques et des anti corps anti borrelia dans le LCR. Nous avons calculé l’AI de tous les patients et procedé à un diagnostic final . Les patients ont été ensuite divisés en trois groupes( neuroborréliose,suspicion de neuroborreliose, temoins).
Resultats : 30 des 40 patients atteints de neuroborreliose avaient un test positif( IA specificité=75%) , 2 des 74 patients avec d’autres desordres neurologiques avait un IA positif( IA specificité=97%)
Conclusion : L’index de synthese intrathecal a une très bonne specificité mais une sensibilité très moyenne. Etant donné le manque de criteres consensuels pour la neuroborrelioseet l’absence de test diagnostic « gold standard » , nous proposons un critere diagnostic pragmatique pour la neuroborreliose, c’est à dire la presence de quatre de ces cinq proprietes : Ne pas avoir d’antecedant de neuroborreliose, avoir une serologie Elisa positive dans le LCR, avoir un index de synthese intrathecal positif, avoir une réaction favorable aux traitements d’antibiotiques specifiques, et ne pas avoir de diagnostic differentiel. Ces nouveaux criteres necessiternt d’être testés sur une échelle plus étendue
